Job Description

Austa Care, under GCAA Health, seeks a Chinese-speaking Case Manager to lead end-to-end home aged care services for SAH clients in Sydney or Melbourne. You will oversee intake, assessments, care planning, service authorisation, and risk management while guiding junior staff and support workers.

Applicants need 2–3 years of case management experience in Australian home aged care, a relevant Diploma or higher, and fluency in Mandarin.
