Who we are

Tweed Coast Support Services is an NDIS provider for people with psychosocial, neurological and neurodivergent diagnoses, and people on SIL funding living in their own homes.

We're not a big national provider. We're a small, local team, and that's exactly why we can take the time to really get to know the people we support- their rhythm, their routines, what matters to them. Small team, big heart- it's how we work, and it's the kind of team you'd be joining.

About the role

We're looking for an experienced Support Coordinator to join us one day a week. It's a small commitment to begin with, but as we grow, so can the role.

You'll work with participants to get the most out of their NDIS plans: connecting them with the right providers and allied health professionals, helping them understand what their funding can actually do, and keeping an eye on how things are tracking against their goals.

The role is mostly work-from-home, with travel to see participants across the Gold Coast and Northern Rivers.

What you'll be doing

Coordinating supports

Help participants and their families find, access and coordinate the services they need

Get the supports in a participant's NDIS plan up and running

Build participants' own capacity to coordinate their supports and take part in their community

Make sure participants genuinely understand their plan and how to use it

Working with others

Build solid working relationships with providers,



allied health and other stakeholders

Reporting and review

Review plans regularly and track progress against goals

Keep accurate records and get reporting to the NDIS in on time

About you

You'll bring hands-on support coordination experience with you. Because it's one day a week, you'll be working fairly independently — so a good working knowledge of the NDIS and comfort with managing your own workload will set you up well.

Demonstrated experience as a Support Coordinator — this is essential

Solid working knowledge of the NDIS, and confidence navigating plans and funding

Robust communicator — in person, on the phone, and in writing

A problem solver who can think on their feet

Comfortable working autonomously and as part of a team

What you'll need

Demonstrated support coordination experience

Experience working with people with psychosocial, neurological and neurodivergent diagnoses

NDIS Worker Screening Check (or willing to obtain before starting)

Completion of NDIS mandatory training (or willing to complete)

A valid driver's licence and reliable, insured vehicle

Preferred

Relevant qualifications in disability services, social work or a related field
