BRIEF DESCRIPTION:

- Fixed-term Parental Leave Cover (Shift Worker) until 30 June 2027
- Full time – Monday, Tuesday and Friday standard operating hours (9:00am – 5:00pm), Wednesday and Thursday evening operating hours (12:00pm – 8:00pm)
- SACSE Level 3 / $81,726 - $87,633 a year + shift worker allowance
- Preston based, Hybrid working arrangements available

YOUR NEW ROLE:

Reporting to the Team Leader Assessment and Planning, as an Intake and Assessment Worker, you will play a vital role in delivering high-quality support to carers. As part of the role, you will work a variety of shifts including evening operating hours shifts from 12:00pm – 8:00pm on set rostered days. As an Intake and Assessment Worker, you will undertake holistic assessments, develop goal-directed care plans and coordinate with carers to improve carers' quality of life and strengthen their caring relationships.

YOU WILL BE RESPONSIBLE FOR:

As an Intake and Assessment Worker, you will:

- Undertake assessments to help carers access supports or to link them in with other services.
- Undertake risk assessments,



organise respite for carers utilising federal/state funding and liaise with service providers to facilitate other longer term supports.
- Undertake review of care plans to ascertain if support needs have been met and follow up accordingly.
- Assist with handling inbound and outbound calls and attend to client queries sent via email or client management system.
- Maintain comprehensive and up-to-date records and case notes in the client management system according to service standards and practices.

WHAT YOU NEED TO SUCCEED:

- Degree or Diploma in Social Work, Community Services or equivalent discipline.
- Minimum of two years’ experience in similar role.
- Demonstrated capacity to deal with individuals, families and communities with diverse and complex cultural, emotional and social needs.
- Solid understanding of holistic, person-centred assessments, goal planning and service coordination.
- Sound knowledge of the community care sector and appropriate referral pathways.
- Excellent organisational and interpersonal skills, adaptability and enthusiasm.
