As our services continue to grow, we are seeking experienced Lifestyle Support Workers to join our team and deliver safe, respectful and person-centred support across our NDIS and Aged Care services.

About the Role

As a Lifestyle Support Worker, you will work alongside our participants to support their independence, choice, wellbeing and participation in the community.

You will follow each participant’s individual support plan and work collaboratively with participants, families, carers and other members of our team.

Your responsibilities may include:

- providing personal care and assistance with daily living activities;
- supporting our participants to access and participate in the community;
- assisting our participant to attend appointments, activities and social events;
- providing domestic assistance, including meal preparation and household tasks;
- assisting with medication in accordance with participant support plans and Livable’s policies and procedures;
- supporting our participants to work towards their individual goals;
- building our participants’ confidence, independence and capacity;
- identifying and reporting changes in a participant’s needs, wellbeing or circumstances;
- completing accurate and timely case notes and other required documentation;
- signing in and out of each rostered shift using Livable’s systems;
- following participant support plans, risk assessments, behaviour support plans and workplace procedures;
- maintaining professional boundaries and participant confidentiality; and
- building positive, respectful and skilled relationships with our participants, families, carers and other members of our team.

Lifestyle Support Workers may be required to work a range of shifts, including mornings, afternoons, evenings, overnights (sleepover and active), weekends and public holidays, depending on participant needs and contracted arrangements.

About You

- We are looking for people who are:




- experienced in disability support, aged care or a related community services role;
- committed to delivering safe, respectful and person-centred support;
- reliable, punctual and available to attend their rostered shifts;
- respectful of each participant’s rights, dignity, privacy, choices and individual needs;
- compassionate, professional and committed to maintaining appropriate boundaries;
- confident working independently in participants’ homes and within the community;
- able to work collaboratively with our participants, families and other members of our team;
- effective communicators who can adapt their approach to meet individual participant needs;
- committed to following participant plans, policies, procedures and lawful and reasonable directions;
- able to complete accurate case notes and other required records before the end of each shift; and
- comfortable using digital systems for rostering, communication, sign-in and sign-out, and participant documentation.

Essential Requirements: To be considered for a Lifestyle Support Worker position with Livable, you must have:

- Certificate III or higher in Individual Support, Ageing, Disability, Aged Care, Community Services or another relevant qualification;
- demonstrated experience working in disability support and/or aged care;
- current First Aid Certificate;
- current CPR certification;
- current NDIS Worker Screening Check;
- current Working with Children Check;
- completion of the NDIS Worker Orientation Module, Quality, Safety and You;
- current Australian driver licence;
- access to a reliable and comprehensively/third party insured motor vehicle;
- a smartphone and the digital literacy required to use Livable’s workplace systems; and
- current Australian working rights.

Successful applicants must also be willing to:

- complete Livable’s pre-employment screening requirements;
- participate in Livable’s induction program;
- complete all mandatory training within the required timeframes;
- maintain all required checks, clearances and certifications;




- comply with Livable’s policies, procedures and Code of Conduct; and
- perform the inherent requirements of the role, with reasonable adjustments considered where required.
