Novi Care Group is healthy growing disability support organisation focused on providing thoughtful, person-centred services that help people with disability live with greater choice, independence and control.
We’re looking for an addition to our already fantastic team - an experienced Support Coordinator and or coordinators to join our team and work across Support Coordination, allied health and participant support networks.
This role is suited to someone who understands that achieving good outcomes isn't about coordinating one service in isolation. It requires looking at the whole person, understanding their needs and goals, and bringing the right people and services together.
The person/s we're looking for
We’re looking for someone who is experienced, compassionate, organised and genuinely interested in people.
Someone who can build trust with a participant, have a professional conversation with an OT, advocate with a provider and confidently coordinate a multidisciplinary team - all while keeping the participant's goals at the centre.

About the role
You will manage your own participant caseload while working closely with participants, families, support workers, allied health professionals,
providers and other key stakeholders.
You will play an important role in making sure supports are well coordinated, communication is clear and participants have access to the services they need.
Requirements for the Role
- Current and valid NDIS Worker Screening Check
- Current Working with Children Check
- Current Police Check
- Resume outlining relevant experience
- Professional references relevant to the role
- Demonstrated NDIS Support Coordination experience
- NDIS Worker Orientation Module completion
- Current First Aid & CPR (desirable/required depending on your role and duties)
- Current driver's licence and reliable vehicle if the role involves participant/community visits
Your responsibilities will include:
- Understanding the NDIS, its guidelines and ethical framework.
- Managing an allocated caseload and building genuine participant relationships.
- Understanding participant goals, needs and support requirements.
- Supporting participants to implement and maximise their NDIS plans.
- Coordinating Support Coordination and allied health services.
- Working collaboratively with allied health professionals and support providers.
- Coordinating multidisciplinary teams, care meetings and referrals.
- Identifying gaps in support and proactively finding solutions.
- Monitoring funding and supporting informed budget decisions.
- Advocating for participant choice, control and independence.
- Supporting NDIS plan reviews and changes in circumstances.
- Liaising with the NDIA, plan managers, providers, allied health, guardians and families.
- Completing reports, documentation and high-quality case notes.
- Identifying and escalating risks and safeguarding concerns appropriately.
- Following through on actions and ensuring supports are progressing.
- Working collaboratively with the Novi Care team to achieve meaningful participant outcomes.

$35–$45 per hour superannuation, depending on experience and qualifications.
