Solace Coordination is seeking an experienced NDIS Support Coordinator to join our team in a flexible hybrid role, working from our Nerang office and your home office.

We are a specialised Support Coordination service with a strong focus on supporting people living with neurological, neurodegenerative, complex and progressive disabilities. Our work is highly collaborative and often involves complex support needs, multidisciplinary teams, Home & Living arrangements, hospital discharge planning and navigating changes in participants' circumstances.

About the Role

As a Support Coordinator, you will work closely with participants, families, informal supports, providers and allied health professionals to implement NDIS plans and coordinate supports that respond to each participant's individual goals and circumstances.

Your responsibilities will include

- Building trusted relationships with participants and their families
- Supporting participants to understand and implement their NDIS plans
- Connecting participants with suitable providers, allied health professionals and community supports
- Monitoring NDIS budgets and supporting effective use of funding
- Coordinating multidisciplinary teams and stakeholder meetings
- Identifying and responding to risks, service gaps and changes in circumstances
- Supporting plan reassessments, variations and change-of-situation requests
- Coordinating supporting evidence and reports
- Liaising with the NDIA, plan managers, providers and mainstream services
- Supporting complex transitions, including hospital discharge and changes in living arrangements
- Maintaining accurate and timely case notes and documentation
- Advocating for participant choice, control and access to appropriate supports

About You

We are looking for someone who is organised, compassionate, proactive and confident working independently. Ideally, you will have:

- Experience working as an NDIS Support Coordinator
- A sound understanding of the NDIS and participant funding




- Strong written and verbal communication skills
- Excellent organisation and time-management skills
- Confidence communicating with participants, families, allied health professionals and service providers
- The ability to manage competing priorities and complex situations
- Solid skilled boundaries and sound judgement
- Good documentation and case-management skills
- The ability to work independently while contributing to a collaborative team
- A genuine commitment to participant choice and control

Experience supporting people with neurological or neurodegenerative conditions, complex disability, Home & Living, hospital discharge or high-risk situations will be highly regarded. Relevant qualifications in disability, community services, social work, allied health or a related field will also be viewed favourably.
