Job Summaryn
n The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. n Prepare patient for patient examination and treatmentn

n Perform tasks within the framework of supportive care, under the clinical direction of licensed health care provider or registered skilled nursen
n Support self-care , self- management and the established plan of care.n
n Utilize standard precautions and practice at all timesn
n Maintain equipment, supplies, inventory, and a safe and clean work environmentn




n Maintain technical knowledge by attending educational workshops, mandatory in-services; reviewing publications and participating in department committee meetings or work groupsn
n Engage in quality improvement initiatives including continuing education, participation in system-wide and department specific teamsn
n Maintain a cooperative relationship among the healthcare team by effectively communicating informationn
n Respond to patient requests, communicates effectively to the licensed health care provider who can fulfill the patient requestn
n Education/Experience Requirementsn
n Required:n
n Graduate of an accredited school of licensed practical nursing programn
n Preferred:n
n Bachelors or Masters of Science in Nursing from accredited nursing programn
n Licensure/Certification Requirementsn
n Required:n
n Current New York State licensure as a Licensed Practical Nurse;n
n Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire daten
