Program provides social work and discharge planning services to hospital patients and their families to improve patient outcomes and support safe transitions from hospital to appropriate care settings. Team members at Care Guidance work closely with health and aged care professionals to deliver compassionate, person-centered support.

Role Description This is a casual, on-site Social Worker role based in Traralgon, VIC.

The Social

Worker will provide person-centered support to older adults and their families across aged care , focusing on emotional wellbeing, practical guidance, and safe transitions of care. Day-to-day responsibilities may include conducting psychosocial assessments, delivering counseling sessions, and coordinating with multidisciplinary teams within aged care homes. The role involves advocating for clients, connecting them with appropriate community and aged care resources, and documenting assessments, plans,



and progress in line with qualified and organizational standards.

The Social

Worker will also contribute to continuous improvement of services by identifying needs, sharing feedback, and collaborating with colleagues and partner providers.

Qualifications

- Formal qualification in Social Work (Bachelor’s or Master’s degree) and eligibility for membership with a relevant professional body.
- Demonstrated experience in community, or aged care social work, including psychosocial assessment and discharge planning.
- Strong skills in counseling, crisis intervention, and person-centered support for older adults and their families.
- Effective communication and documentation skills, including the ability to engage sensitively with diverse individuals and stakeholders.
- Ability to collaborate within multidisciplinary teams and coordinate services across hospital, aged care, and community providers.
- Sound knowledge of aged care systems, community services, and relevant legislation or guidelines related to older people’s care.
- Organizational and time management skills to handle multiple cases, prioritise tasks, and meet reporting requirements.
- Commitment to ethical practice, cultural safety, and respect for people of all backgrounds, identities, and abilities.
- Proficiency with basic digital tools and case management or health record systems; willingness to learn new systems as needed.
