About the role

As a Social worker, you will provide culturally protected, trauma informed, and client centred psychosocial assessment, counselling, case management, support and advocacy for Aboriginal and/or Torres Strait Islander clients, families, and communities accessing FPHW. This is an experienced practitioner role requiring sound independent clinical judgement and the ability to manage complex and high-risk presentations. The role operates within a holistic model of health that recognises the interconnected nature of physical, social, emotional, cultural, and spiritual wellbeing, and works collaboratively as part of a multidisciplinary care team.

Key responsibilities

- Provide culturally safe, responsive and trauma-aware psychosocial assessment, counselling, advocacy, and case management for Aboriginal and/or Torres Strait Islander individuals, families, and communities.

- Deliver holistic support informed by Aboriginal concepts of social and emotional wellbeing, recognising connections to family, kinship, community, culture, Country, and spirituality, alongside the social determinants of health.

- Support clients experiencing family violence, trauma, grief and loss, housing instability, financial hardship, involvement with child protection or justice systems,



and other psychosocial concerns or barriers to health and wellbeing.

- Develop, implement, and regularly review individualised, strengths-based care plans in partnership with clients and, with the client's consent, their families, carers, and the FPHW multidisciplinary team.

- Provide brief interventions and short-term counselling, and facilitate culturally appropriate referrals and warm handovers to specialist or external services where required.

- Undertake psychosocial risk assessments, safety planning, and appropriate escalation in response to concerns including family violence, suicide, self-harm, child safety, and other risks, consistent with professional, legal, and organisational requirements.

- Advocate alongside and on behalf of clients when engaging with external agencies, including but not limited to Centrelink, housing services, child and family services, justice services, and the NDIS.

- Coordinate and communicate with FPHW clinicians and external services to support continuity of care, while maintaining informed consent, privacy, and confidentiality.

- Deliver services in accordance with Aboriginal and/or Torres Strait Islander cultural protocols, community priorities, and the principles of self-determination, cultural safety, and Aboriginal community control.

- Build respectful relationships with community members, Elders, and local Aboriginal organisations, where appropriate and guided by FPHW's cultural governance and community engagement processes.

- Participate in community events, outreach, health promotion, and service-development activities as required.

- Maintain accurate, timely, and confidential clinical documentation, and contribute to relevant service reporting, evaluation, and quality-improvement activities.

- Participate in qualified supervision, reflective practice, continuing professional development, and multidisciplinary case consultation.

- Work within professional scope of practice, applicable legislation and standards, and FPHW policies and procedures.

Essential

- Bachelor/Masters of Social Work degree and eligibility for AASW membership

- Minimum 3 years' post qualification experience in a social work role.

- Genuine interest in and commitment to working with Aboriginal and/or Torres Strait Islander peoples, and an understanding of the social and emotional determinants of health in these communities.

- Well developed assessment, case management, counselling, and brief intervention skills.

- Demonstrated sound understanding of a broad range of theoretical frameworks, and ability to provide culturally safe, trauma informed, and person/family centred care.

- Ability to work both independently and collaboratively within a multidisciplinary team.

- Excellent interpersonal and communication skills, with the ability to engage effectively across diverse ages, cultures, and social backgrounds.

- A commitment to practice in accordance with the AASW code of ethics 2020, scope of practice, professional standards and organisational policies and procedures.

- Strong computer literacy and time management skills.

Desirable

- Experience using the Best Practice software.

- Experience working in community based, primary health care, or Aboriginal Community Controlled Health settings.

- Understanding of the unique health needs and experiences of Aboriginal and Torres Strait Islander peoples, including intergenerational trauma.

- Experience in family violence, child protection, mental health, alcohol and other drugs, or aged care contexts.

- Experience with telehealth and remote service delivery.

- Completion of cultural safety or Aboriginal health training.
