Vision and Care is a registered NDIS provider delivering personalised disability and community support services across Adelaide and South Australia. We are committed to assisting individuals to achieve greater independence, participate in their communities and access appropriate services and resources.

We are currently seeking a dedicated and experienced Community Worker to join our team.

Key Responsibilities

The successful applicant will be responsible for:

- Assessing the needs of clients and identifying appropriate community, health, welfare, housing and social support services.
- Supporting clients to identify their individual goals and develop strategies to improve their independence and community participation.
- Providing information, guidance and referrals to appropriate community services and support organisations.
- Liaising with community organisations, welfare agencies, government bodies, health professionals and other service providers on behalf of clients.
- Assisting clients to access suitable community programs, educational activities, employment services and recreational opportunities.
- Developing and maintaining relationships with local community organisations and service providers.
- Coordinating community-based activities and programs that promote social inclusion and participation.
- Monitoring clients' progress and reviewing their changing support and community service needs.
- Advocating for clients and assisting them to overcome barriers to accessing community resources and services.
- Maintaining accurate client records,



case notes and reports in accordance with organisational and regulatory requirements.
- Contributing to the development and improvement of community programs and services.
- Promoting awareness of available community resources and support services.

Skills and Experience

The successful candidate will have:

- Relevant qualifications in Community Services, Social Services, Welfare, or a related field.
- Relevant experience in community services, disability services, welfare or a similar environment.
- Strong communication and interpersonal skills.
- Ability to assess client needs and coordinate appropriate services and referrals.
- Ability to communicate effectively with clients, families, community organisations and other service providers.
- Positive organisational, documentation and case management skills.
- A person-centred and culturally sensitive approach.
- Ability to work independently and as part of a multidisciplinary team.

Additional Requirements

The successful applicant may be required to hold or obtain:

- NDIS Worker Screening Check
- Working with Children Check
- National Police Clearance
- Current Australian driver's licence
- First Aid and CPR certification
