We are seeking an experienced and passionate Registered Nurse to join our team as a Care Partner (Clinical).

This is an exciting opportunity to combine your clinical expertise with a person-centred approach to care, supporting older people to remain independent, and well-supported in their own homes.

As a Care Partner (Clinical), you will work closely with a team of Care Partners, customers, families, and healthcare professionals to provide clinical guidance and oversight for customers with more complex care needs. You will play a key role in supporting positive customer outcomes while helping to build the clinical capability of the team.

About the Role

Working collaboratively with a team of Care Partners, you will assist in managing customers with complex health conditions, support care planning, and ensure clinical risks are identified and managed appropriately.

Key responsibilities include but are not limited to:

- Conduct assessments, reviews, co design and update care plans identifying clinical and care needs.
- Collaborate with a dynamic multidisciplinary team to provide comprehensive support to older individuals.
- Support customers with complex clinical needs and chronic health conditions.
- Review hospital discharge summaries and support in coordinating post-discharge care requirements.
- Assisting with palliative and end-of-life care coordination.

- Identify, document, manage and escalate clinical deterioration,



risks, and safeguarding concerns.
- Provide clinical advice, education, and practical support to Care Partners and other team members.
- Support customers and families to understand clinical recommendations, risks, and available care options.
- Liaise with GPs, specialists, hospitals, pharmacists, allied health professionals, and community services.
- Contribute to clinical governance, continuous improvement, incident review, and evidence-informed practice.
- Maintain accurate, timely and defensible clinical documentation.
- Deliver services in accordance with professional standards, organisational policies, aged care requirements, and scope of practice.

- Build effective relationships with external stakeholders, including carers, medical professionals, and community services.

About You

To be successful in this role, you will have:

- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA).
- Demonstrated clinical experience, preferably within aged care, community care, chronic disease management, or care coordination.
- Strong assessment and clinical decision-making skills.
- Experience supporting customers with complex care needs.
- Excellent communication and relationship-building skills.

- Sound knowledge of contemporary aged care standards, clinical governance, risk management, and professional nursing obligations.
- Ability to work autonomously, exercise sound clinical judgement and prioritise competing clinical demands.
- Strong clinical documentation, care planning, and risk assessment skills.

- The ability to work collaboratively within a multidisciplinary team.

- Commitment to maintaining professional development and evidence-informed practice.

Current driver's licence as you will be required to travel within the community, as well as a National Criminal History Record Check.
