? Support Partner – Help Older Australians Stay Independent (Hybrid Role) ?

South Melbourne | Full Time | 3 days in office

What you’ll be doing

- Supporting older Australians to stay independent at home

- Working with customers and families to build straightforward,



practical care plans

- Helping people understand and use their Home Care Package funding

- Running Telehealth check-ins, reviews, and support calls

- Keeping things organised, compliant, and running smoothly behind the scenes

What we’re looking for

- Around 1+ year experience in aged care (Care Manager, Case Manager, Coordinator or similar)

- A good understanding of Home Care Packages

- Comfortable using systems and typing while talking on calls

- Someone calm, organised, and easy to talk to

- Most importantly — someone who genuinely cares about helping people
