We are looking for an experienced In-Home Complex Care Coordinator to join our team and take a lead role in coordinating high-quality, person-centred supports for participants with complex care needs. This is a hands-on coordination role suited to someone with strong experience in complex disability support, NDIS requirements and the coordination of safe, high-quality care.

What You'll Do

- Coordinate and oversee in-home complex care supports, ensuring services are safe, consistent and aligned with participant needs
- Work closely with participants, families, support workers, nurses, allied health professionals and other stakeholders
- Develop and maintain solid participant relationships and ensure support plans are followed
- Monitor participant health, wellbeing, risks and changes in support needs
- Provide guidance and support to care teams working with participants with complex needs
- Ensure participant documentation, progress notes, care plans and other records are accurate and maintained
- Identify and respond to risks, incidents and changes in participant circumstances
- Work with senior management to identify service improvements and manage emerging issues
- Ensure compliance with NDIS Practice Standards, organisational policies and relevant legislation

What You'll Bring

- Minimum 3 years' experience in complex care, disability, community health,



aged care or a similar care environment
- A nursing qualification and current skilled registration is highly preferred, particularly for candidates with experience in complex health care, clinical risk management or high-needs disability support
- Strong experience coordinating supports for people with complex health or disability needs
- Strong understanding of the NDIS and relevant quality and safeguarding requirements
- Excellent communication, documentation and stakeholder management skills
- Ability to work independently, prioritise competing demands and make sound decisions
- A practical, participant-focused approach and strong attention to detail

Requirements & Checks

- Minimum 3 years' relevant experience

Minimum Certificate IV qualification in Disability, Community Services or a related field. A nursing qualification with current professional registration is highly preferred.

- Current NDIS Worker Screening Check
- Current Working With Children Check
- Current driver's licence and access to a reliable vehicle
- Right to work in Australia
