Join the General Practice Liaison Officer team and use your clinical expertise to strengthen connections between primary care and Metro South Health, improve outpatient pathways and enhance the patient journey.

Your new role:

As a Clinical Nurse within the General Practice Liaison Officer Program, you will play a key role at the interface between primary, community and secondary care. Working autonomously, you will provide expert clinical advice, coordination and problem-solving to support General Practice and Metro South outpatient services. You will build strong relationships with clinicians and stakeholders, identify opportunities for improvement and provide clinical leadership, education and support to enhance healthcare integration and patient outcomes.

You will,

- Provide a key contact point for General Practice to navigate Metro South outpatient services and referral processes.
- Strengthen communication and coordination between primary and secondary care to enhance the patient journey.
- Build and maintain relationships with primary care clinicians through consultation, liaison, practice visits and resource coordination.

About you:

You are an experienced Registered Nurse with advanced clinical knowledge, strong problem-solving capability and the ability to build effective relationships across healthcare settings. You will bring a collaborative approach, confident communication skills and an understanding of General Practice, specialist outpatient services and referral pathways.



Your ability to lead change and work across organisational boundaries will be highly valued.

- Advanced clinical, critical thinking, planning and coordination skills.
- Strong knowledge of General Practice, primary care, specialist outpatients and referral processes.
- Excellent communication, consultation, negotiation and stakeholder engagement skills.

The Princess Alexandra Hospital (PAH) is a tertiary health care centre providing care in most major adult specialties.



We provide world class health care to Brisbane City's southern suburbs across emergency, medical, mental health and surgical services, including liver and kidney transplant.

We are nationally recognised for our expertise in trauma management and organ transplants and are one of Australia's leading academic and research health centres. The PAH as a major medical research precinct hosts Australia's most advanced medical research facility, the Translational Research Institute (TRI) as well as a new Clinical Research Facility for the discoveries made at TRI to be trialed in a safe clinical setting.
