Monash Health in Melbourne, VIC, invites applications for a Grade 3 Social Worker in the Youth Mental Health Unit 2 at Dandenong Hospital. The role is full time, Monday to Friday, focusing on young people aged 18–25 with acute and complex mental health needs.

You will assess, intervene, advocate and plan discharges within a multidisciplinary team. You will lead complex discharge planning, work with families, coordinate services (housing, NDIS, AOD, family violence) and supervise junior social
