Which brings us to you. We're after an experienced support worker who leads by doing, who's just as comfortable delivering exceptional care to participants while also leading a team of Support Workers who are responsible for keeping the lives and homes of our participants running smoothly. This isn't a role where you sit back and direct from a distance; it's hands-on, on-the-ground leadership where your presence and consistency genuinely shape the experience of every participant and Support Worker on your sites.

About the role
n
As Support Leader, you'll provide day-to-day support to participants along with on-site leadership to Support Workers, ensuring services are delivered safely, consistently, and in line with participant needs and plans. You'll be the visible, steady presence across your sites - supporting your team, championing quality outcomes for participants. This role is based between Wynyard to Latrobe.

Day to day, you will:

Deliver hands-on, person-centred support to participants across your sites
n
Provide leadership, and coaching to Support Workers across your allocated sites
n
Lead by example, embedding LEAP's values and person-centred practice in everything your team does
n
Ensure care plans, behaviour support plans, and documentation are current, understood, and followed
n
Maintain a visible, hands-on presence across your sites and varying shifts
n
Coordinate participant appointments and keep relevant documentation accurate and up to date, communicating changes to Support Workers
n
Own accurate, timely reporting, including medication management and end-of-month documentation
n
Identify and elevate risks, incidents, and participant wellbeing concerns as they arise
n
Seek guidance & input from Supervisor to ensure timely updates are maintained across all sites
n
Build genuine, trusted relationships with participants and their families
n
Be a reliable contributor to the organisation's after-hours on-call roster
n

About you
n
You're a natural leader who thrives on the frontline. You'll bring:
n
n
Certificate IV in Disability, Community Services, or equivalent (or working towards)
n
Demonstrated experience in disability support with complex needs
n
Strong communication skills and a genuine, person-centred approach
n
The ability to manage competing priorities, while remaining calm under pressure
n
A current driver's license, and the ability to obtain (or hold) a NDIS Worker Screening Check, Working with Children Check, and vulnerable people registration
n
