Job Description
Western Sydney Local Health District is seeking a Transition and Youth Health Project Officer to lead the implementation of the Transition Care Strategy and provide strategic support to Youth Health Services.
You will coordinate projects, engage stakeholders, and translate data insights into service improvements to benefit young people with chronic conditions during their transition from paediatric to adult care.
