Albury Wodonga Health is seeking an Aged Service Emergency Team Care Coordinator (CNS or Allied Health) to lead assessment, care planning and coordinated care for older patients in the Emergency Departments.
The role supports clinical pathways and evidence-based practice across acute and sub-acute settings. AHPRA registration and a post-graduate qualification are required, with at least five years of clinical experience and demonstrated leadership.
