Clinical pharmacy services in a tertiary hospital

Duties tailored to experience - dispensary support and/or ward-based clinical care
n
Areas may include general medicine, surgical, rehab, or oncology
n

Key Responsibilities:
n
n
Dispensary Services: Accurately review and process medication orders, counsel patients, and support medication safety initiatives.
n
Ward-Based Clinical Care: Participate in multidisciplinary ward rounds, manage medication histories, monitor therapies, and facilitate safe discharge processes.
n
Team Collaboration & Development: Work closely with colleagues, mentor junior staff and students, and contribute to pharmacy education and quality improvement projects.
n
