We are seeking a passionate Nurse to coordinate and facilitate consumer assessments to support AN-ACC classifications and requests for reclassification, as indicated by change in clinical condition of consumers within our Residential Aged Care facilities.

Tallangatta Health Service is a 61 bed, small rural public health service providing a mix of acute, sub-acute, aged care residential, and community services to Tallangatta and the surrounding district. We pride ourselves on being a consumer-focused progressive provider of care to our community, patients and residents with a multidisciplinary focus.

About you
n




You are passionate about person-centred care and motivated to enhance the wellbeing of our consumers. You will also meet the following criteria:
n
n
Current registration as a Registered or Enrolled Nurse with AHPRA.
n
Extensive knowledge and experience with provision of care within an aged care facility.
n
Demonstrated ability to perform initial and ongoing assessments, to support AN-ACC classifications and request/s for reclassification as indicated by change in clinical condition of consumers.
n
Demonstrated commitment to person-centred care.
n
Well-developed interpersonal and communication skills.
n
Strong time management and problem-solving abilities.
n
Commitment to continuous improvement.
n
Computer literacy with word-processing and data base programs
n
n
All appointments are made subject to the required security screening including National Criminal History and an immunisation status appropriate to the role, including the annual Influenza vaccination.
