Amplar Health, part of the Medibank Group, is seeking an Enrolled Nurse Care Partner to deliver person-centred care in a home-based setting.
You will assess, plan and coordinate services with a Senior Care Partner (RN) to support independence and healthy ageing for consumers.
You'll work within a multidisciplinary team to monitor wellbeing, intervene early, and help clients stay connected and thriving at home.
