As a Care Partner , you'll support customers throughout their care journey, helping them achieve their goals and remain living safely and independently at home or in a place of their choosing.
Working closely with customers, families, clinical teams and external providers, you'll develop holistic care plans, coordinate services and ensure customers receive person-centred support.
About the role: This is a Permanent Full-Time opportunity offering flexibility and a outstanding work-life balance.
Flexible work hours : 8:30am – 4:30pm or 9:00am – 5:00pm, Monday to Friday Attractive remuneration -Up to $***** per hour plus superannuation Hybrid role combining client visits, Branch office days (Dubbo- 3 days)
