We are seeking an experienced and compassionate NDIS Support Coordinator to join our team.

About the Role
n
As a Support Coordinator, you will support NDIS participants to understand and implement their plans, connect with suitable providers and build their confidence to manage their supports.
n
You will maintain positive relationships with participants, families, support providers, allied health professionals and other stakeholders.
n
Key Responsibilities
n
n
Help participants understand and implement their NDIS plans.
n
Identify suitable services and connect participants with quality providers.
n
Build participants' capacity to manage their supports independently.
n
Coordinate services across disability, health, housing and community sectors.
n
Monitor participant budgets and ensure effective use of NDIS funding.
n
Conduct regular participant reviews and respond to changing support needs.
n
Prepare progress reports, review reports and supporting documentation.
n
Assist participants with NDIS plan reassessments and change-of-circumstances requests.
n
Manage risks, service gaps, incidents and complex situations appropriately.
n
Maintain accurate case notes and records in accordance with NDIS requirements.
n
Develop strong relationships with participants, families, providers and community organisations.
n
Build referral networks and contribute to the continued growth of SAN Support.
n
Maintain appropriate professional boundaries and participant confidentiality.
n

About You
n
The successful candidate will ideally have:
n
n
Previous experience in NDIS Support Coordination or a similar role.
n
A good understanding of NDIS plans, budgets and funding categories.
n
Knowledge of the NDIS Practice Standards and participant safeguarding requirements.
n
Strong communication, relationship-building and problem-solving skills.
n
The ability to manage a participant caseload and meet reporting deadlines.
n
Strong organisational and time-management skills.
n
Confidence working independently and as part of a team.
n
Experience supporting participants with complex needs will be highly regarded.
n
Relevant qualifications in disability, community services, social work, psychology, allied health or a related area will be advantageous.
n

Essential Requirements
n
n
Current NDIS Worker Screening Check.
n
Working with Children Check, where required.
n
Current Australian driver's licence.
n
Access to a reliable and insured vehicle.
n
Working rights in Australia.
n
Ability to complete the NDIS Worker Orientation Module.
n
Satisfactory reference and employment checks.
n
