Tender Loving Care Aged Care – Bankstown NSW

As a Care Partner, you will support Paramedical Healths customers throughout their care journey, helping them achieve their goals and remain living safely and independently at home or in a place of their choosing. Working closely with customers, families, clinical teams and external providers, you'll develop holistic care plans, coordinate services and ensure customers receive person-centred support. This is a Permanent Full-Time prospect, Monday to Friday, supporting SAH customers.

Key responsibilities

Build trusted relationships with customers and support them to achieve their goals at home.

Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.

Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.

Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.

Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.

Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.

Promote safety, quality and person-centred care across all aspects of service delivery.

About you

Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.

Strong customer focus, with the ability to build trusted relationships and navigate sensitive conversations with empathy and professionalism.

Excellent communication skills and confidence using Microsoft Office and customer management systems.

Knowledge of funding streams, quality standards, audit requirements and care transitions, including discharge planning.

Robust decision-making, problem-solving, time management, and prioritisation skills in a quick-paced environment.

A team-oriented mindset, with the ability to work effectively across multidisciplinary teams, families, clinicians and external providers.

Relevant qualifications in Community Services, Aged Care or a related discipline (Certificate IV or Diploma desirable).
