Job Description
Townsville Hospital and Health Service is seeking a Discharge Coordinator for the Adult Acute Mental Health Inpatient Unit (AAMHIU) at Townsville University Hospital.
You will coordinate discharges and work with the CNC and Shift Coordinator to optimise bed capacity within the 38-bed service.
You will engage with consumers, families and carers, review cases, support warm handovers and ensure documentation supports protected, continuous care.
