Clinical Care Partner for Support at Home provider
n
Are you an Allied Health or Registered Nurse qualified who is ready to change career paths and join a not-for-profit organisation which specialises in the delivery of Home Care Support across the Eastern Suburbs area Sydney. Draw on your previous experience working with patients within a clinical setting and adapt those skills to our community clients.

Responsibilities include:
n
Proven experience working with complex clients, needs assessments and risk/safety assessments
n
Create and maintain the client's case management plan including conducting assessments, and determining the services needed in collaboration with the client.
n
Advocate on behalf of our client to a variety of organisation and ensure that all services are deliverable against their government funding
n
Develop, implement, monitor and review case plans in collaboration with theclient and families
n
Provide comprehensive assessments to identify client needs and requirements for clients using internal and external service networks.
n
Following up on leads that are generated through word of mouth, marketing and other initiatives.
n

To be successful you must:
n
Have experience and an understanding of Support at Home Program
n
Have a tertiary qualification in either Nursing, Allied Health, Disability or Gerontology
n
Be organised and have strong attention to detail
n
Strong written and verbal communication
n
Customer focused attitude and a passion for making a difference with the vulnerable and elderly.
