HammondCare is seeking a permanent part time Clinical Partner – Registered Nurse to support older adults at home across the Central West/Nepean/Blue Mountains region.
The role involves home visits, group activities, and delivering person-centred care within a multidisciplinary team.
Three days per week, with hybrid working options and travel between client homes.
You will be supported by experienced clinicians and contribute to high-quality home-care services in Australia.
