Nepean Blue Mountains Local Health District is seeking a Clinical Nurse Specialist Grade 2 to lead community health services from Lemongrove Community Health Centre. You will provide expert clinical leadership, mentor nursing staff, and deliver client-centered care in home and community settings.

The role focuses on managing chronic and complex conditions at home, coordinating with GPs and allied health specialists, and using evidence-based guidelines to improve outcomes.
