Step Up Supports is seeking a Client Clinical Case Manager (Registered Nurse preferred) for a full time role in Ballarat. You will provide clinical oversight for client medical management and support staff through complex situations in the community and home settings.
You'll participate in a rotating after-hours roster, including one evening per week and rotating weekend coverage, working with clients, families and the broader support team to achieve positive outcomes.
