We are seeking anexperiencedDisabilityServices Managerwith solid Sector experience and a solid understanding of the NDIS.
About the role
This is a hands-on, people-focused role coordinating participant supports and working closely with participants, families, Support Workers and other professionals.
You'll take the time to listen to people, understand what is important to them and respond to their individual needs, while ensuring services are well coordinated and delivered in line with NDIS requirements.
Key responsibilities
Managing the Police HomeCare journey for your own portfolio of Clients, from onboarding through to exit stage.
Build strong relationshipswith participants, familiesand Support Workers.
Listen to participantsand understand theirindividual needs, preferencesand goals.
Coordinate and monitorparticipant supports.
Regularly check inwith participants andensure their needsare being met.
Supervise, support & guideSupport Workers.
Assist with participant onboarding and Support Worker matching.
Maintain participant plansand documentation.
Respond to feedback, concerns and complaints.
Support incident, riskand safeguarding processes.
Work with families,



Support Coordinators and alliedhealthprofessionals.
Monitor service qualityand ensure NDIScompliance.
Ensuring our services are delivered in line with NDIS requirements, the NDIS Practice Standards and PoliceHomecare Services policiesand procedures.
Assisting with theongoing growth anddevelopment of ourorganisation and team.
About you

1+ year experience in a similar role, preferably within disability, community services, agedcare or arelated sector.
Strong understanding oftheNDIS and PracticeStandards.
Excellent communicationand interpersonal skills.
Strong organisational andtime-management skills, whilst being able to work independently or as part of a team.
Current driver's licenceand reliable vehicle.
Current NDIS WorkersScreening/Clearance
Current Working with ChildrenCheck.
First Aid and CPR orwillingness to obtain.
Relevant qualificationsin Disability, CommunityServices, Management ora related fieldare highly regarded.
