This role focuses on supporting students throughout their transition journey, particularly during their first 90 days of study, while working collaboratively within a "Team Around the Learner" framework.What you'll do:Provide individualised transition support to students with disabilityDevelop transition plans and coordinate tailored support strategiesAssist students and families to navigate enrolment and student support processesFacilitate meetings with schools, families, allied health professionals and Institute staffCollaborate with internal support services to improve student outcomesBuild relationships with secondary schools and external disability support networksMaintain accurate records, reporting and program dataContribute to best-practice disability transition initiatives across the Victorian TAFE NetworkWhat You'll BringExperience supporting people with disability, mental health conditions and/or medical conditionsUnderstanding of disability-related legislation, reasonable adjustments and inclusive practiceExcellent communication and stakeholder engagement skillsStrong case management, planning and organisation abilitiesAbility to build effective relationships with students, families, schools and internal stakeholdersA commitment to improving educational outcomes for people with disabilityExperience within education, disability, youth,
