Duties:
Conduct triage, comprehensive assessments, risk assessment, and safety planning.
Deliver short-term case management and assertive outreach.
Provide brief psychoeducation and therapeutic interventions.
Collaborate in care planning and discharge with internal and external stakeholders.
Practice in accordance with Trauma-Informed Care and Recovery principles
The successful canidate will possess:
Relevant qualification in Social Work.
Eigibility for membership with the AASW.
Experience in mental health clinical or community settings.
Strong assessment and case management skills.
Experience with CIMHA database preffered.
Catagory A immunisations.
