About the role

Miwatj Health Aboriginal Corporation is seeking an experienced and passionate Chronic Conditions Program Lead to provide leadership, strategic direction and support for the delivery of chronic and complex conditions care across East Arnhem Land.

This role will work across Miwatj communities, including the Gove Peninsula, Galiwin’ku, Milingimbi, Gapuwiyak and Ramingining, with a robust focus on improving health outcomes for Yolŋu through culturally safe, integrated, evidence-based and sustainable models of care.

This position requires regular travel to remote communities across East Arnhem Land, including travel by small/light aircraft and 4WD.

What you’ll do

As the Chronic Conditions Program Lead, you will provide expertise and operational support to Primary Health Care teams and chronic and complex care clinicians across Gove Peninsula, Galiwin’ku, Milingimbi, Gapuwiyak and Ramingining.

You will support workforce development and orientation, chronic disease programs, quality improvement, clinical governance and multidisciplinary care. You will also build relationships with internal and external stakeholders, visiting specialists and allied health providers to improve coordination and continuity of care.

The position also has responsibility for chronic disease reporting and action planning, supporting program funding opportunities, contributing to policy and practice governance, and overseeing budgets for funded chronic and complex care programs.

About you

We are looking for someone who brings:





- A minimum of 5 years’ broad nursing experience post-registration
- Demonstrated experience in chronic disease program delivery at a population level
- Experience working with Yolŋu people and within the Aboriginal Community Controlled Health sector
- Experience in continuous quality improvement within Primary Health Care
- Strong knowledge of chronic disease, remote health and the social determinants of health
- Highly developed communication, stakeholder engagement, leadership and change-management skills
- A commitment to regular travel to remote communities by 4WD or light aircraft.

Relevant tertiary/postgraduate experience in chronic disease-related areas and a current NT-recognised Immunisation Provider Certificate are required. Experience as a Remote Area Nurse and the ability to speak Yolŋu Matha and/or knowledge of Yolŋu kinship systems are desirable.
