St Vincent's Hospital Melbourne is seeking a Grade 2 Allied Health Qualified or nursing equivalent to join the Transition Care Program. The role offers part-time 0.6 EFT backfill until 31 August 2027, with a focus on comprehensive case management for aged care clients within a supportive multidisciplinary team.

The Case Manager will conduct holistic assessments, coordinate care, monitor outcomes and facilitate appropriate links to hospital and community services.
