Moran Aged Care is seeking a dedicated Care Planning Coordinator to join the Clinical Leadership team at Moran Home in Kellyville. This Monday to Friday in office role offers the chance to work closely with clinical leaders, influence resident outcomes and contribute to best-practice care in a supportive environment.
Reporting to the Care Manager, you will oversee the overall clinical assessment program, support RNs to complete assessments, and help tailor care plans to each resident’s needs.
