Omnia Community Health is seeking an Enhanced Care Coordinator to join our multidisciplinary counselling team in the Enrich Program. You will provide assessment, intervention and referrals for young people aged 12–25 with moderate to severe mental health concerns, managing a diverse caseload in Sunbury and Macedon Ranges.
The role involves face-to-face work in a variety of settings, collaboration with other health professionals, and delivering therapeutic interventions and case coordination
