Mackay Hospital and Health Service is seeking experienced senior allied health professionals to join the Aged Care Assessment Team on a temporary basis. You will assess aged clients across community and hospital settings and develop personalised Support Plans with recommended care pathways.

Role involves liaising with clients, carers, and other health professionals, using IAT and My Aged Care portals to ensure accurate data and outcomes. Check the attached Role Description for details.
