Axil Health is seeking a Clinical Pharmacist to lead Chronic Care Management, Transition Care Management, and Remote Patient Monitoring across our clinic network. You will establish skilled relationships with the medical team, manage patient care, and travel as needed within the region.

You will enroll patients, oversee device management, interpret RPM data, and collaborate on medication management with supervising providers.
