We work in partnership with community, Elders, service providers and stakeholders to support healing, connection and holistic wellbeing.

We are seeking a passionate and dedicated Aboriginal Health Practitioner to join our team and support the delivery of culturally safe, holistic healthcare for Aboriginal and Torres Strait Islander people across South Western Sydney.

n

This rewarding role will focus on supporting clients through health assessments, care coordination, advocacy, education, and culturally responsive health promotion activities. You will play a key role in strengthening access to healthcare services while supporting improved health outcomes for community.

About the Role n

As an Aboriginal Health Practitioner, you will:

- Conduct and initiate Aboriginal and Torres Strait Islander mental health assessments and reviews.
n
- Provide follow-up support, care coordination and advocacy for clients.
n
- Maintain accurate clinical records and data entry.
n
- Deliver health promotion, education, counselling, group programs
n
- Support the delivery of Social, Emotional and Wellbeing programs.
n
- Support groups, excursions, cultural activities and community events.
n
- Participate in multidisciplinary case conferences and care planning.
n
- Provide culturally appropriate training and cultural awareness presentations to external agencies and service providers.
n
- Promote improved access to mainstream health services for Aboriginal and Torres Strait Islander people.
n




- Represent Gandangara Health Services on relevant committees, networks and community forums.
n

About You n

This is an Identified Aboriginal Position . Applicants must be of Aboriginal and/or Torres Strait Islander descent and be able to demonstrate Aboriginality. This position is exempt under Section 14(d) of the Anti-Discrimination Act 1977 (NSW).

To be successful in this role, you will also have:

n

n
- A strong commitment to improving Aboriginal and Torres Strait Islander health outcomes.
n
- Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care Practice (or equivalent recognised qualification).
n
- Experience working in a primary healthcare or community health setting.
n
- Demonstrated experience delivering training, education or group programs.
n
- Sound understanding of Aboriginal cultural practices, values and community issues.
n
- Excellent communication, interpersonal and relationship-building skills.
n
- Strong organisational skills with the ability to manage competing priorities.
n
- Experience working collaboratively within multidisciplinary teams.
n
- Proficiency in Microsoft Office and electronic clinical record systems.
n
- A current NSW Driver Licence.
n
- A current Working with Children Check and willingness to obtain a National Police Check.
n
