Mounties Care Home is seeking a dedicated Care Partner for a full time role at our Hope Island, QLD office.
You will coordinate care, help clients stay independent at home, and oversee a team of Carers to ensure quality services.
Key duties include developing care plans, budgeting, accessing My Aged Care, community visits, and engaging with potential client leads.
You'll also participate in after-hours on-call rosters and maintain confidentiality.
