Job Description

Lives Lived Well in Chinchilla is seeking an AOD Counsellor Case Manager to deliver evidence-based treatment and care coordination across rural communities. You will manage intake, counselling, case planning and referrals while building strong partnerships with local services in the Western Downs region.

Based in Chinchilla with outreach to Miles and Tara, the role offers a fixed-term contract until 30 June 2027, 38 hours per week, and a market-competitive salary plus superannuation and salary
