Job Description

Endeavour Foundation is seeking a Client Intake and Assessment Practitioner in Cannon Hill to be the first point of contact for Home and Community enquiries, guiding clients to suitable Endeavour services and supports.

You will analyse disability assessments, conduct screenings, and tailor support plans to match clients with appropriate services based on their needs and funding. Grow partnerships across Qld, NSW and VIC.
