About the Role

n

We are seeking compassionate, motivated and patient-focusedRegistered Nursesto join ourGeriatric Evaluation and Management (GEM) Rehabilitation Ward.

n

This is an excellent opportunity for a Registered Nurse who is passionate about providing high-quality care to older people and supporting patients to regain their independence, confidence and quality of life following illness, injury or hospitalisation.

n

As a Registered Nurse, you will provide high-quality, person-centred nursing care while working closely with patients, families and the multidisciplinary team.

n

Key responsibilities

n

n
- Providing safe, evidence-based and person-centred nursing care.
n
- Completing comprehensive nursing assessments and developing individualised care plans.
n
- Monitoring patients' clinical condition and responding appropriately to changes in health status.
n
- Supporting patients with rehabilitation, mobility, activities of daily living and reablement goals.
n
- Working collaboratively with physiotherapists, occupational therapists, allied health professionals, medical staff and other members of the multidisciplinary team.
n
- Supporting medication management and other clinical interventions.
n
- Educating patients and families to promote self-management and independence.
n




- Contributing to effective discharge planning and transition of care.
n
- Maintaining accurate and timely clinical documentation.
n
- Participating in quality improvement, education and clinical governance activities.
n
- Promoting dignity, respect, choice and positive patient experiences.
n

What we're looking for

- Current AHPRA registration as a Registered Nurse.
n
- Strong clinical assessment and nursing skills.
n
- Excellent communication and interpersonal skills.
n
- A commitment to person-centred and evidence-based care.
n
- The ability to work effectively within a multidisciplinary team.
n
- A positive, adaptable and collaborative approach.A genuine interest in supporting older patients to achieve their rehabilitation goals.

About the GEM Rehabilitation Ward

n

Our GEM rehabilitation service provides specialised multidisciplinary care to older patients with complex medical, functional and rehabilitation needs.

n

Working as part of a collaborative multidisciplinary team, you will support patients throughout their rehabilitation journey, with a strong focus onreablement, independence, mobility and safe discharge planning.

n
