Job Description
IPC Health is seeking an experienced Strengthening and Restoring Family Services Practitioner to support families across Brimbank, Melton and Western Melbourne.
The role involves assertive, relational case management for expectant parents, infants and families across Strengthening Families and Restoring Families streams.
You will undertake comprehensive assessments, develop care plans with SMART goals, and work within multidisciplinary teams to promote safety and family stability.
