Job Description
Department of Justice and Community Safety VIC – Melbourne VIC is seeking an Assessment and Transition Co-ordinator to support people through their prison journey and plan for release.
You will assess needs, coordinate services with Corrections Victoria Reintegration Pathway, develop transition plans and parole applications, and work with providers to support people leaving prison.
Robust attention to records and reporting is required.
