Job Description

This multidisciplinary role focuses on expert screening, comprehensive assessment and care planning for patients over 60 to reduce readmissions and ED re-presentations.
Eligible applicants include Registered Nurses and Allied Health specialists with APHRA registration, postgraduate qualification, and at least five years of relevant clinical experience.
