Job Description

GV Health in Victoria's Community Health locations is seeking a Service Support Coordinator - HCP/NDIS. This permanent part time role (64 hours per fortnight) supports Aged Care clients to manage their Support At Home funds, coordinating supports and liaising with Care Managers.

You will help connect clients to services, monitor satisfaction, and work within a vibrant team to maintain independence in their homes within the Community Interlink partnership.
