In your new role you may be allocated to any hospital ward or clinical specialty within the cairns facilities to deliver social work clinical services to patients/carers and families.

HITH provides care in a patient's permanent or temporary residence for conditions that require ongoing clinical governance, monitoring and/or treatment, as a substitution for in-hospital admitted care.

About your opportunity:

In this position you will….

- Independently apply professional judgement and decision making to manage a caseload, adapting routine and non-routine clinical practice based on valid and reliable clinical evidence, with clinical practice supervision, commensurate with experience.
- Conduct comprehensive psychosocial assessments,



develop appropriate intervention plans, and achieve outcomes, informed by social work theory and contemporary knowledge of social determinants of health; medical terminology; legislation; resources; services and policy requirements.
- Actively participate in and contribute to the business arrangements of the Hospital in the Home service including team meetings; quality improvement; safety and clinical governance activities; and development of policies and procedures.
- Provide advocacy to address client needs and ensure equity and access to services.
