Job Description

VincentCare Victoria is seeking a Team Leader - HomeConnect to oversee a team of case managers and multidisciplinary staff. You will lead service delivery aimed at preventing homelessness in private rental and community housing sectors, with a focus on person-centred support.

Reporting to the Manager - HomeConnect, you will guide staff, manage brokerage, compile data and reports, and ensure funding and service agreements are met while building robust partnerships with external agencies.
