As a Support at Home Care Coordinator, you'll play an important role in coordinating personalised support for our clients and working closely with clients, families, support workers and external providers.

Key responsibilities

Coordinating and monitoring client support services

Building positive and respectful relationships with clients and their families

Communicating with support workers and subcontractors to ensure services are delivered effectively

Coordinating appointments, services and client requirements

Liaising with allied health professionals and other service providers

Monitoring client needs and arranging appropriate supports

Completing client documentation, care plans and administrative requirements

Maintaining accurate records and ensuring compliance with relevant requirements

Responding to client and family enquiries in a timely and professional manner

Supporting clients to achieve their individual goals and maintain independence

Caring, approachable and genuinely enjoys working with older people





Highly organised with excellent attention to detail

Confident communicating with clients, families and a range of professionals

Able to manage competing priorities and work independently

Qualified, reliable and proactive

Comfortable using technology and various computer systems

A strong problem solver who can think on their feet

Respectful of individual choices, preferences and independence

Ability to operate in a fast-paced environment
